Trustee's speech under fire: NoHum Union board to meet on controversy

The Northern Humboldt Union High School District board will hold a special meeting Friday to address the actions of trustee Dan Johnson, who apparently passed off a well-known 2012 speech as his own letter to his daughter at the Arcata High School graduation in June.

The 9 a.m. meeting is scheduled to take place at the Arcata High School multi-purpose room.

Johnson could not be reached for comment.

?At this time, the comment is we're having a meeting to discuss it as it's written in the agenda, and we'll follow the action item when we get there,? Superintendent Chris Hartley said. ?We just want to have the meeting, and let the board have a chance to address the issue.?

Board member Dana Silvernale said the board initially held off on discussing the matter because Hartley went on vacation immediately following Arcata High's commencement ceremony. Silvernale said the board wanted to wait to address the matter until Hartley returned.

She said she anticipated the board would discuss the matter directly with Johnson and Hartley in closed session before deciding what, if any, action would be appropriate.

The Arcata Eye reported late last month that Johnson called his daughter up to the podium and read a 2012 speech given by Massachusetts' Wellesley High School English teacher David McCullough, Jr., without attribution.

McCullough's speech, which the Boston Globe said ?railed against the coddling of children and rallied Wellesley High's graduating seniors to seize life, read, and live selflessly,? became an Internet sensation when it went viral last year. The Boston Globe reports it also resulted in the teacher receiving a book contract.

The agenda item related to the speech reads: ?Possible response to public concern regarding the 2013 Arcata High School graduation ceremony and accept Board member Dan Johnson's statement to community.? It is listed as an action item.

If you go:

What: Commencement speech meeting

Who: Northern Humboldt Union High School District board

Where: Arcata High School multi-purpose room

When: Friday, 9 a.m.

Could This New Approach To Cancer Cure The Disease Altogether?

Apart from AIDS, finding a cure for cancer is the Philosopher's Stone of modern medicine. Now, in research laid out by Harvard University - one of the most respected universities in the world - there may be hope for finally creating a cure for the disease.

According to Sciencedaily, Martin Nowak, a professor of mathematics and of biology and director of the Program for Evolutionary Dynamics, and co-author Ivana Bozic, a postdoctoral fellow in mathematics, show that, "under certain conditions, using two drugs in a "targeted therapy" -- a treatment approach designed to interrupt cancer's ability to grow and spread -- could effectively cure nearly all cancers."

    cancer drug

In effect, the two-drug approach would stop the cancer from growing and spreading.

Top 5 Cancers Among Men And Mortality Rates:

"In some sense this is like the mathematics that allows us to calculate how to send a rocket to the moon, but it doesn't tell you how to build a rocket that goes to the moon," Nowak said. "What we found is that if you have a single point mutation in the genome that can give rise to resistance to both drugs at the same time, the game is over. We need to have combinations such that there is zero overlap between the drugs."

Top 5 Cancers Among Women And Mortality Rates:

At present, most cancers are treated with single drug therapies, so the idea of a two-drug step is pioneering. The thinking is that with single drug therapies, the cancer can mutate and develop a resistance to the drug, whereas in two-drug therapies, the thinking is that the "cancer needs to make two independent steps" to develop and therefore may fail.

"I think we can be confident that, within 50 years, many cancer deaths will be prevented," Nowak added. "One hundred years ago, many people died from bacterial infections, and now they would be cured. Today, many people die from cancer, and we can't help them, but I think once we have these targeted therapies, we will be able to help many people -- maybe not everyone -- but many people."

Last week, human trials for a 'wonder drug' that could shrink tumours began, which had been developed by the University of Stanford. That particular drug works by boosting the immune system to break down cancerous tumours.

Other developments in the field of surgery have also been encouraging. Imperial College London unveiled a 'cancer sniffing knife' called the iKnife, which could detect cancerous and non-cancerous tumours, leading to fewer complications and more effective surgery.

Small Businesses Created 45 Percent of the New Jobs in June

    new jobs

Small businesses continue to spur U.S. job growth. Companies with fewer than 50 employees created 45 percent of the 188,000 new jobs in June, a recently released ADP National Employment Report said.

The 84,000 jobs created by small firms represent nearly half of the jobs created by U.S. businesses and the percentage of jobs created by small companies continues to grow.

This is the second consecutive month for growth in job creation among smaller firms. In May, small businesses created 63,000 jobs, up from 57,000 in April. That month saw a major decline from March?s 72,000 jobs added by smaller companies.

Smallest of the Small Carry the Load

Also, as in previous months, the smallest of the small represented the majority of small business job creation, the ADP Small Business Report said. Between May and June, 54,000 jobs were created by businesses with less than 20 employees. By comparison, businesses with 20-49 employees added 31,000 jobs during the same period.

The results are part of an ongoing trend. In May, companies with between 1 and 20 employees created 37,000 of the 58,000 jobs credited to small businesses. And in April, businesses in the same category created 34,000 of the 50,000 jobs added by small businesses as a whole.

    Where Job Growth Occurred

As usual the largest job growth occurred in the service sector with small businesses creating about 70,000 of these jobs.?The sector includes jobs pouring lattes but there are many other kinds of jobs and businesses represented as well.

The service sector includes financial, professional and business services, but also includes employees like restaurant workers, housekeepers, teachers, health care workers and retail sales workers too.

June also saw the largest increase of jobs in the goods producing sector in four months with 27,000 new jobs added, 14,000 among smaller companies. For small businesses, this sector includes mainly construction and manufacturing.

Obamacare Not Affecting Job Growth ? Yet

It?s obvious that companies with fewer than 50 employees continue to represent a growing percentage of jobs being created. Fifty full-time employees is the number at which businesses will be required under the new Patient Protection and Affordable Care Act to provide medical benefits or pay a penalty per employee for not offering them by Jan. 1, 2015.

Still, in a prepared statement issued with the report (PDF), Mark Zandi, chief economist of Moody?s Analytics, insisted:

Health Care Reform does not appear to be significantly hampering job growth, at least not so far.

Has your small business added any jobs in the last few months? ?Do you have any plans to do so in the near future?

Verizon leak shows VZ Edge upgrade program: trade your phone in once you've paid off half of it

Verizon leak shows VZ Edge upgrade program trade your phone in once you've paid off half of it

T-Mobile's been grabbing a lot of headlines lately with its consumer-friendly handset deals and upgrade programs, but it appears that Big Red has similar ideas. The folks at Droid Life have acquired a Verizon document detailing a new VZ Edge program that'll allow folks to trade in their phones once they've paid off 50 percent of the purchase price. To join the program, Verizon will merely require a monthly fee of unknown amount, and in return, folks can get their upgrades early fee free and sans contract. When will customers get the option to do so? According to the doc, launch is scheduled for Q3, August 25th, to be exact. So, if Verizon can stick to its schedule, its customers are just under six weeks away from being free from the tyranny of the new every two upgrade cycle.

CHINA: 'After Earth' Looks to Change its Fortunes with $13.8M Opening Weekend

AE-4.jpgA crash-landing at the North American box office might have been unexpected for After Earth, but there always the hope that the film could change its luck overseas. Will Smith and son/co-star Jaden took off on a lengthy worldwide promotional tour for the film as Sony waited anxiously for the global numbers to come in. After Earth hit 5,218 screens in China six weeks after it opened in its first overseas market, South Korea. The sci-fi flick closed its run in South Korea with $3.4 million, well below the $10.1 million earned by Oblivion in the same territory. If the $197 million overseas cume from Tom Cruise sci-fi vehicle is used as a reference point, then After Earth would need a big push from its Chinese release to help mitigate the damage suffered at the North American box office.

A $13.89 million opening weekend was enough for? After Earth?to claim the top spot in China's weekly box office chart, according to figures from EntGroup. The film's total represents three days of release in the territory and accounts for 2,587,773 admissions. The China premiere allowed the film to break the $150 million overseas mark, giving it a $157.5 million overseas total and a $216.8 million global cume. If it can hold well in the coming weeks,?After Earth?might be able to reach a healthy $35 million total in the market.

Sci-fi has proved popular in China all summer. The market represents nearly a quarter of the $222.5 million overseas total from Star Trek Into Darkness ($57M) and became the only overseas territory where Oblivion broke the $20 million mark, bringing in $23.7 million of the Tom Cruise vehicle's $197 million overseas gross.

China also has the reputation of "saving" several recent Hollywood box office disasters. Rob Cain, an authority on the Chinese film industry, previewed the film's opening in the territory with a think-piece considering After Earth's box office potential in China. Cain points out China's role in helping recent North American fiascos like John Carter and Battleship cross the $200 and $300 million global marks, respectively. John Carter grossed $73.1 million in North America and was rescued by a $42.1 million China run that took its global total to $201 million. Battleship fared better overseas, posting a mere $65.4 million in North America and a $50.1 million total from China that raised its global tally to $303 million.

Johnnie To's Blind Detective finished its first full week of release in second place with a $12.62 million total to bring its Chinese cume up to $26.55 million. Already a smash hit, Tiny Times recently announced an August 9 release for a sequel. That's less than two months after the premiere of the current box office hit, which has grossed $75.73 million in 18 days. Man of Steel is closing out its run in China, ending the week with a $4.17 million take. The Superman reboot has grossed a total of $62.51 million in China. Man of Tai Chi, the Chinese co-produced directorial debut of Keanu Reeves, fell flat in its premiere last weekend. This week's numbers were equally as poor; Man of Tai Chi earned $1.18 million in its first full week in theaters to take its Chinese total to $4.28 million.

Greenwald: Snowden docs contain NSA 'blueprint'

RIO DE JANEIRO (AP) ? Edward Snowden has very sensitive "blueprints" detailing how the National Security Agency operates that would allow someone who read them to evade or even duplicate NSA surveillance, a journalist close to the intelligence leaker said Sunday.

Glenn Greenwald, a columnist with The Guardian newspaper who closely communicates with Snowden and first reported on his intelligence leaks, told The Associated Press that the former NSA systems analyst has "literally thousands of documents" that constitute "basically the instruction manual for how the NSA is built."

"In order to take documents with him that proved that what he was saying was true he had to take ones that included very sensitive, detailed blueprints of how the NSA does what they do," Greenwald said in Brazil, adding that the interview was taking place about four hours after his last interaction with Snowden.

Snowden emerged from weeks of hiding in a Moscow airport Friday, and said he was willing to meet President Vladimir Putin's condition that he stop leaking U.S. secrets if it means Russia would give him asylum until he can move on to Latin America.

Greenwald told The AP that Snowden has insisted the information from those documents not be made public. The journalist said it "would allow somebody who read them to know exactly how the NSA does what it does, which would in turn allow them to evade that surveillance or replicate it."

Despite their sensitivity, Greenwald said he didn't think that disclosure of the documents would prove harmful to Americans or their national security.

"I think it would be harmful to the U.S. government, as they perceive their own interests, if the details of those programs were revealed," said the 46-year-old former constitutional and civil rights lawyer who has written three books contending the government has violated personal rights in the name of protecting national security.

He has previously said the documents have been encrypted to help ensure their safekeeping.

Greenwald, who has also co-authored a series of articles in Rio de Janeiro's O Globo newspaper focusing on NSA actions in Latin America, said he expected to continue publishing further stories based on other Snowden documents over the next four months.

Upcoming stories would likely include details on "other domestic spying programs that have yet to be revealed," but which are similar in scope to those he has been reporting on. He did not provide further details on the nature of those programs.

Greenwald said he deliberately avoids talking to Snowden about issues related to where the former analyst might seek asylum in order to avoid possible legal problems for himself.

Snowden is believed to be stuck in the transit area of Moscow's main international airport, where he arrived from Hong Kong on June 23. He's had offers of asylum from Venezuela, Nicaragua and Bolivia, but because his U.S. passport has been revoked, the logistics of reaching whichever country he chooses are complicated.

Still, Greenwald said that Snowden remains "calm and tranquil," despite his predicament.

"I haven't sensed an iota of remorse or regret or anxiety over the situation that he's in," said Greenwald, speaking at a hotel in Rio de Janeiro, where he's lived for the past eight years. "He's of course tense and focused on his security and his short-term well-being to the best extent that he can, but he's very resigned to the fact that things might go terribly wrong and he's at peace with that."

Greenwald said he worried that interest in Snowden's personal saga had detracted from the impact of his revelations, adding that Snowden deliberately turned down nearly all requests for interviews to avoid the media spotlight.

Asked whether Snowden seemed worried about his personal safety, Greenwald responded, "he's concerned."

He said the U.S. has shown it's "willing to take even the most extreme steps if they think doing so is necessary to neutralize a national security threat," Greenwald said. "He's aware of all those things, he's concerned about them but he's not going to be in any way paralyzed or constrained in what he thinks he can do as a result of that."

Asked about a so-called dead man's pact, which Greenwald has said would allow several people to access Snowden's trove of documents were anything to happen to him, Greenwald replied that "media descriptions of it have been overly simplistic.

"It's not just a matter of, if he dies, things get released, it's more nuanced than that," he said. "It's really just a way to protect himself against extremely rogue behavior on the part of the United States, by which I mean violent actions toward him, designed to end his life, and it's just a way to ensure that nobody feels incentivized to do that."

He declined to provide any more details about the pact or how it would work.

Greenwald said he himself has beefed up his own security, particularly since a laptop went missing from his Rio home.

"I don't really feel comfortable discussing the specific measures, but one would be really irrational and foolish to have thousands of top-secret documents from the most secretive agency of the world's most powerful government and not be thoughtful about added security," he said.

It was not immediately clear whether Russia would take Snowden up on his latest request for asylum, which could further test U.S.-Russia relations.

Following Friday's meeting between Snowden and human rights activists, U.S. officials criticized Russia for allowing a "propaganda platform" for the NSA leader.

White House spokesman Jay Carney said Russia should instead send Snowden back to the U.S. to face the felony charges that are pending against him.

Carney said Snowden is not a human rights activist or a dissident. "He is accused of leaking classified information, has been charged with three felony counts and should be returned to the United States," the spokesman said.

    2010-2013 Volkswagen Golf Pre-Owned

Model: 2010 to 2013 Volkswagen Golf
    Vehicle Type: Hatchback

The Verdict: Decent reliability, an upscale driving experience, and plenty of selection in the used market mean used Golf shoppers should have no problem confidently finding a model to suit their needs.

Price: Pricing on this generation Golf varies widely based on numerous trim grades and powertrain options. A newer, low-mileage unit can command in excess of $22,000, possibly more with the diesel engine. Older, higher-mileage units can be found from the low- to mid-teens.

What Owners Like: This era of Golf is rated highly in terms of comfort, utility, ride quality, and an overall upscale and sporty driving experience. Many owners say they love the Golf?s cabin, which is attractively styled, highly functional, and nicely trimmed.

What Owners Dislike: Typical complaints include high up-front pricing, wind noise at speed, and a mediocre, standard stereo system. On models with larger wheels, expect a rougher ride, too.

History/Description: The 2010 to 2013 Volkswagen Golf delivered style and comfort with an eye for versatility, flexible operation, and plenty of selection in the marketplace. Countless variants of the popular German hatchback were available, including a sporty GTI model, a high-performance R variant, and an ultra-thrifty TDI.

Our focus here will be on the mainstream models powered by the ?standard? gasoline engine or the popular TDI diesel engine. Gas models got a 2.5L 5-cylinder with 170 horsepower. The TDI engine was a 2.0L unit with 140. Look for manual, automatic or DSG gearboxes, depending on the model selected.

Golf was available in 3- or 5-door configurations, with Trendline, Comfortline, Sportline, and Highline models spanning basic to loaded, respectively.

Available feature content included Bluetooth, leather seating, a sunroof, premium audio, auto climate control, and more.

'Buddhist bin Laden' or man of peace? Anti-Muslim monk stirs controversy in Myanmar

Paula Bronstein / Getty Images, file

Wirathu is the leader of a Buddhist extremist movement known as 969. He was once jailed for anti-Muslim violence and believes it is vital that Buddhist people keep their business, relationships and land dealings away from followers of Islam.

By Fiona MacGregor, NBC News contributor

YANGON, Myanmar --?A monk has been dubbed the "Buddhist bin Laden" and the "face of Buddhist terror" after launching a campaign against Muslims.

U Wirathu has been accused of inciting?violence against Myanmar's Muslim minority?with fiery sermons claiming the growth of Islam is putting Buddhism and Burmese culture at risk.

About 200 people have been killed by violence since religious riots erupted in June 2012 and tens of thousands fled after homes owned were burned by mobs.

Wirathu, who says he once compared himself to the al Qaeda leader as "a joke," claims?Islamic beliefs can encourage people to have "bad characters." As a key figure in a movement that encourages people to boycott Muslim businesses, Wirathu espouses a form of radical Buddhism which seems incongruous to those in the West who associate the religion with peace and acceptance.

However, the 46-year-old ?monk insists he does not advocate violence against Muslims, who make up just 5 percent of the country's 60 million people.

Soe Zeya Tun / Reuters, file

A Buddhist monk and other protesters demonstrate against TIME magazine in Yangon, Myanmar, on June 30.

"I believe Islam is a threat not just for Buddhism, but for the people and the country and the religion," he said.

According to Wirathu, it doesn't matter that Muslims make up a small proportion of Myanmar's population.?

"When someone went into a school in Virginia armed and killed people there, it didn't matter that the teacher and the pupils were the majority," he offers as an illustration.

Last month, TIME magazine's portrayal of Wirathu as "The Face of Buddhist Terror" sparked widespread protests across the country.

But Wirathu laughs at reports that he even branded himself the "Buddhist bin Laden."

"People used to write things like that about me on Facebook, call me that, and the 'bald Bin Laden,' all sorts of names," he said. "I ended up calling myself that as a joke ... and it got reported from there."

In Myanmar, Buddhist religion and culture means monks are generally seen as beyond question because of their holy status, yet at the same time their role has long been political as well as spiritual.

Wirathu's notoriety has grown amid deadly?violence between Buddhists and Muslims in Myanmar, which?is in?its second year on "the road to democracy"?after decades of military rule.

Tensions are highest in the western state of Rakhine, which borders predominantly Muslim Bangladesh. Rakhine has long been the scene of conflict between rival ethnic groups but sectarian violence has increasingly been breaking out in other parts of the country. In May, Buddhist mobs burned Muslim homes in the northern city of Lashio.

Human Rights Watch noted that violence in June and October 2012 "resulted in countless deaths, destruction to property, large scale internal displacement and segregation within Rakhine state of Myanmar."

Other organizations have recorded incidents of mosques and Muslims being attacked in Rakhine state and elsewhere. Tens of thousands of Muslims have been effectively trapped in displacement camps after clashes saw an estimated 200 people killed and?villages destroyed?last year.

Wirathu is behind a pro-Buddhist movement, known as 969. Its badge has been appearing on businesses keen to promote their Buddhist credentials, and encourage other Buddhists to choose theirs over Muslim-run ones. The logo also increasingly appears at rallies and on pamphlets issued by religious and nationalist organizations describing Muslims as a threat.

Wirathu claims he came up with the idea of 969, numbers which represent Buddhist virtues.

He insists it is vital that Buddhist people keep their business, relationships and land dealings away from Muslims because he fears "their population will get bigger than ours" and "shariah law will take over."

Wirathu and others operating under the 969 umbrella highlight videos and images of butchered monks, violent crowds of people in Muslim dress and destroyed temples which they allege show attacks by Islamists targeting Buddhists.

Wirathu insisted he "does not believe that violence will solve the problems" and suggested many of the more inflammatory quotes attributed to him have either been taken out of context or are simply wrong.

"I don't believe that violence is the way forward and I do not want to see Buddhist people ganging up and taking revenge on Muslims," Wirathu said.?

To most Buddhists in Myanmar, the idea that a monk could be responsible for inciting violence is almost incomprehensible, yet many seem happy enough to accept his message of intolerance.

"The monk Wirathu is peaceful person and he is not a terrorist. Look around the world, at Afghanistan, Indonesia, who are the main culprits when it comes to terrorism?" asked Htway Maung Kyaw, an elderly Yangon bookseller, who having studied in the U.K. in his youth is proud to describe himself as "Philologist (London)"

    Niels Huby

The Venerable Ashinkumara believes Muslims are plotting to take over Myanmar.

"It is true he was in prison," he said of Wirathu's detention in 2003 under the junta, reportedly for incitiing violence against Muslims. "However, that was under the military regime [when many people were imprisoned]. He was struggling on behalf of humanity."

The Venerable Ashinkumara, a fellow monk and member of the 969 movement, said his controversial friend has "a tender heart."

Ashinkumara, who believes that Muslims are plotting to take over Burma and "are the ones who started all the fights," added: "I don't believe Wirathu has called for violence."

This view that Wirathu is somehow "misunderstood" is expressed by businessman and Buddhist charity founder Ko Moe whose maternal grandfather was Muslim and whose grandmother was he says "forced" to convert from Buddhism.

"Wirathu never told people to fight against Islam," Moe said. "He is trying to give the message that people should protect their own religion and race.

"Not to fight, but there is a big concern that the Muslim population is growing faster than the Burmese people, because they can have many wives and are told not to use birth control," he says, echoing a view heard over and over again in Burma's tea-houses.

Moe points to Indonesia, Malaysia and southern Thailand, which are now predominantly Muslim, as examples of what many in Myanmar fear is their country's future.

Many also express the feeling that they are a minority in their region: a nation of 60 million, dwarved by neighbors India at 1.2 billion and China at 1.3 billion, and conscious of the growing influence of Islam in other Asian nations.

Moe's Piti (Light of Truth) charity helps orphans and people living with HIV, but he has clear religious aims and the organization also works to promote "a proper understanding of Buddhism".

"What Wirathu talks about is patriotism," he said. "It is about protecting our nationality, our race and our religion."

For Muslims born in Myanmar, that is a hard view of patriotism to swallow.?

At a central mosque in Yangon, it's not something people like to discuss publicly, but they feel the discrimination against them is about more than religion.

"I don't believe this is really about religion," one imam said. "It is about certain people trying to gain political power. We Muslims have been here for 1,000 years and are Burmese too."

Fed wants more jobs before slowing bond buys

    Economy

U.S.Federal Reserve Chairman Ben Bernanke speaks to the press following the Fed's two-day policy meeting at the Federal Reserve in Washington in June....

    JASON REED / Reuters

U.S.Federal Reserve Chairman Ben Bernanke speaks to the press following the Fed's two-day policy meeting at the Federal Reserve in Washington in June. Meeting minutes indicate that the Fed wants more jobs created before reining in bond buying.

The Federal Reserve wants the job market to improve before it slows the stimulus program that helped speed up the recovery, minutes of the central bank's most recent meeting showed Wednesday.

Even as Fed officials agreed in June about the need to pull back on those economic stimulus measures soon, many wanted reassurance that the employment recovery was on solid ground before a policy retreat.

Financial markets have been wagering on September as the probable start of a reduction in the pace of the U.S. central bank's $85 billion in monthly bond purchases, but minutes of the Fed's June meeting suggested that might not be a sure bet. The bond purchases are aimed at keeping interest rates at historic lows.

"Several members judged that a reduction in asset purchases would likely soon be warranted," the minutes said. But they added that "many members indicated that further improvement in the outlook for the labor market would be required before it would be appropriate to slow the pace of asset purchases."

Global investors have recently recovered from a mild bout of panic that followed Fed Chairman Ben Bernanke's roadmap for an end to so-called quantitative easing, which he said would likely draw to a close by the middle of next year. Financial market fears have been allayed in part by a chorus of Fed officials who have sought to reassure traders that the end of asset buys will not lead to imminent interest rate hikes.

"Many members indicated that decisions about the pace and composition of asset purchases were distinct from decisions about the appropriate level of the federal funds rate," the minutes said.

Whether the markets have gotten the message is not fully clear; the yield on the 10-year U.S. Treasury note has risen a full percentage point in just two months and stands close to its highest levels since 2011.

This has already slowed activity in the mortgage market, which had been key to the recent economic rebound.

At their June meeting, some Fed officials worried not only about the outlook for employment, but the pace of economic growth as well. Many economists believe the economy grew at less than a 1 percent annual rate in the second quarter, although most look for a pick-up in the second half of the year.

"Some (officials) added that they would ... need to see more evidence that the projected acceleration in economic activity would occur, before reducing the pace of asset purchases," the minutes said.

Of the Fed policymakers who argued it would be wise to curtail bond purchases soon, two thought it should be done "to prevent the potential negative consequences of the program from exceeding its anticipated benefits."

AURORA, Colo. (July 10, 2013) Rates of angiography and percutaneous coronary interventions were significantly lower among Medicare Advantage beneficiaries when compared to those covered by Medicare fee-for-service, according to a study by a University of Colorado School of Medicine physician published in the July 10 issue of JAMA.

The study, which included nearly 6 million Medicare Advantage and Medicare fee-for-service beneficiaries from 12 states, also found that geographic variation in procedure rates was substantial for both payment types.

"Treatment of cardiovascular disease is one of the largest drivers of health care cost in the United States, accounting for $273 billion annually. Cardiovascular procedures are major contributors to this high cost," according to background information in the article. "Little is known about how different financial incentives between Medicare Advantage and Medicare fee-for-service (FFS) reimbursement structures influence use of cardiovascular procedures."

"Under the Medicare FFS reimbursement structure, physicians are paid more for doing more procedures. In contrast, integrated delivery systems that provide care for Medicare Advantage beneficiaries receive a capitated payment, and physicians working in these settings are not paid more for doing more procedures," the authors write.

Daniel D. Matlock, MD, MPH, assistant professor of medicine at the University of Colorado School of Medicine, Aurora, and colleagues conducted a study to compare the overall rates and local area rates of coronary angiography, percutaneous coronary intervention (PCI; procedures such as balloon angioplasty or stent placement used to open narrowed coronary arteries), and coronary artery bypass graft (CABG) surgery between Medicare Advantage and Medicare FFS beneficiaries living in the same communities.

The study, which included 878,339 Medicare Advantage patients and 5,013,650 Medicare FFS patients older than 65 years of age, compared rates of these procedures between 2003-2007 across 32 hospital referral regions (HHRs) in 12 states.

The researchers found that compared with Medicare FFS patients, Medicare Advantage patients had lower age-, sex-, race-, and income-adjusted procedure rates for angiography and PCI but similar rates for CABG surgery. There were no differences between Medicare Advantage and Medicare FFS patients in the rates of urgent angiography. When examining procedure rates across HRRs, there was wide geographic variation among Medicare Advantage patients and Medicare FFS patients.
